Background

Pragmatic trials are becoming more widely acknowledged as providing evidence from the real world for clinical decision-making. The term "pragmatic", however, is a word that is often used in contradiction and its definition and measurement require further clarification. The purpose of pragmatic trials is to inform clinical practices and policy decisions rather than confirm a physiological hypothesis or clinical hypothesis. A pragmatic trial should try to be as close as possible to the real-world clinical practice, including recruiting participants, setting, design, delivery and implementation of interventions, determining and analysis results, as well as primary analysis. Methods

In a practical trial the goal is to inform clinical or policy decisions by demonstrating how an intervention would be implemented into routine care. Explanatory trials test hypotheses concerning the cause-effect relationship within idealised settings. Therefore, pragmatic trials could have less internal validity than explanatory trials and may be more susceptible to bias in their design, conduct and analysis. Despite these limitations, pragmatic trials may be a valuable source of information for decision-making in the context of healthcare.

This difference in the primary analysis domain could be due to the fact that the majority of pragmatic trials analyze their data in the intention to treat manner while some explanation trials do not. The overall score for pragmatic systematic reviews was lower when the areas of management, flexible delivery and following-up were combined.

It is important to remember that a pragmatic trial does not necessarily mean a low-quality trial, and in fact there is a growing number of clinical trials (as defined by MEDLINE search, however this is not sensitive nor specific) that use the term 'pragmatic' in their title or 프라그마틱 무료슬롯 abstract. The authors of the Pragmatic Free Trial Meta identified RCTs published from 2022 to 2022 that self-described themselves as pragmatic.
