Pragmatic Free Trial Meta

Pragmatic Free Trail Meta is an open data platform that enables research into pragmatic trials. It collects and distributes clean trial data, ratings, and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses to examine the effect of treatment across trials with different levels of pragmatism.

Background

Pragmatic trials are increasingly acknowledged as providing evidence from the real world for clinical decision-making. The term "pragmatic", however, is not used in a consistent manner and its definition and assessment require clarification. Pragmatic trials are intended to guide clinical practices and policy choices, rather than confirm a physiological hypothesis or clinical hypothesis. A pragmatic study should strive to be as close as it is to actual clinical practices, including recruiting participants, setting, designing, delivery and execution of interventions, determining and analysis results, as well as primary analyses. The most pragmatic trials should not blind participants or the clinicians. This could lead to bias in the estimations of the effect of treatment. Methods

In a pragmatic study, the aim is to inform policy or clinical decisions by showing how an intervention could be implemented into routine care. Conclusions

As the importance of evidence from the real world becomes more commonplace, pragmatic trials have gained traction in research. They are clinical trials randomized that compare real-world care alternatives instead of experimental treatments under development. They involve patient populations that are more similar to the patients who receive routine care, they use comparators that are used in routine practice (e.g. existing medications), and they depend on the self-reporting of participants about outcomes. This method is able to overcome the limitations of observational research for example, the biases that come with the reliance on volunteers, and the limited availability and coding variations in national registries.

Pragmatic trials offer other advantages, including the ability to leverage existing data sources and a higher likelihood of detecting meaningful differences from traditional trials. However, pragmatic tests may be prone to limitations that undermine their reliability and generalizability. For example the participation rates in certain trials may be lower than expected due to the healthy-volunteer effect and incentives to pay or compete for participants from other research studies (e.g., industry trials). Many pragmatic trials are also restricted by the necessity to recruit participants on time. The authors of the Pragmatic Free Trial Meta identified RCTs published up to 2022 that self-described as pragmatic.
