Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that allows research into pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2 permitting multiple and varied meta-epidemiological studies that examine the effects of treatment across trials that have different levels of pragmatism, as well as other design features.
Background
Pragmatic trials are becoming more widely acknowledged as providing evidence from the real world to support clinical decision-making. The term "pragmatic", however, is not used in a consistent manner and its definition and measurement require further clarification. Many RCTs that do not meet the requirements for pragmatism however, they have characteristics that are contrary to pragmatism, have been published in journals of varying types and incorrectly labeled pragmatic. This can lead to false claims of pragmatism, and the use of the term needs to be standardized. The development of a PRECIS-2 tool that provides an objective, standardized evaluation of the pragmatic characteristics is a first step.
Methods
In a pragmatic research study the aim is to inform clinical or policy decisions by demonstrating how an intervention could be integrated into routine treatment in real-world contexts. Explanatory trials test hypotheses regarding the causal-effect relationship in idealized conditions. In this way, pragmatic trials can have a lower internal validity than explanatory studies and are more susceptible to biases in their design as well as analysis and conduct. Despite their limitations, pragmatic research can provide valuable information to make decisions in the healthcare context.
The PRECIS-2 tool scores an RCT on 9 domains, with scores ranging from 1 to 5 (very pragmatist). It is important to improve the quality and accuracy of outcomes in these trials.
Results
While the definition of pragmatism does not require that clinical trials be 100% pragmatist There are advantages when incorporating pragmatic components into trials. These include:

However, pragmatic trials may be prone to limitations that compromise their validity and generalizability. The authors of the Pragmatic Free Trial Meta identified 48 RCTs self-labeled as pragmatic and that were published from 2022.
