Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that facilitates research on pragmatic trials. It collects and distributes clean trial data, ratings and evaluations using PRECIS-2. This permits a variety of meta-epidemiological studies to evaluate the effects of treatment across trials with different levels of pragmatism.
Background
Pragmatic trials are increasingly acknowledged as providing evidence from the real world for clinical decision making. The term "pragmatic" however, is used inconsistently and its definition and measurement require further clarification. Many RCTs that do not meet the requirements for 프라그마틱 pragmatism but contain features contrary to pragmatism, have been published in journals of various types and incorrectly labeled as pragmatic. This could lead to misleading claims of pragmatism, and the use of the term should be standardized. Methods
In a pragmatic study it is the intention to inform policy or clinical decisions by demonstrating how an intervention would be incorporated into real-world routine care. The PRECIS-2 tool assesses the degree of pragmatism in an RCT by assessing it across 9 domains ranging from 1 (very explanatory) to 5 (very pragmatic). Another common aspect of pragmatic trials is that the researchers try to make their results more valuable by studying subgroups of the sample. This can result in unbalanced analyses with less statistical power. This increases the risk of missing or misdetecting differences in the primary outcomes. In the instance of the pragmatic trials that were included in this meta-analysis this was a major issue since the secondary outcomes were not adjusted to account for variations in baseline covariates.
Furthermore, pragmatic trials can also be a challenge in the gathering and interpretation of safety data. This is because adverse events are typically reported by participants themselves and are prone to delays in reporting, inaccuracies, or coding variations. Therefore, it is crucial to improve the quality of outcome ascertainment in these trials, in particular by using national registry databases instead of relying on participants to report adverse events on the trial's own database.
Results
Although the definition of pragmatism may not require that all trials be 100% pragmatic, there are some advantages to incorporating pragmatic components into clinical trials. These include:

The original PRECIS tool3 was an adapted version of the PRECIS tool3 that was based on the same scale and domains. They found that pragmatic reviews scored higher across all domains, however they scored lower in the primary analysis domain.

These terms may indicate that there is a greater awareness of pragmatism within titles and abstracts, but it's not clear whether this is reflected in content.
Conclusions
As the importance of real-world evidence grows widespread, pragmatic trials have gained momentum in research. They are randomized studies that compare real-world care alternatives to clinical trials in development. They include patient populations that are more similar to those who receive treatment in regular care. This approach can help overcome the limitations of observational studies, such as the biases associated with reliance on volunteers and limited accessibility and coding flexibility in national registries.
Other benefits of pragmatic trials include the ability to use existing data sources, as well as a higher probability of detecting significant changes than traditional trials. However, they may be prone to limitations that compromise their credibility and generalizability. Participation rates in some trials may be lower than expected because of the healthy-volunteering effect, financial incentives or competition from other research studies.
