Many serious accidents have occurred because an equipment started running when it was supposed to be turned off for repairs or maintenance. All "affected", "authorized", and "qualified" personnel must be taught about the use of lockout and tagout devices. These devices include, but are not only restricted to locks, chains and padlocks. Each device is identified and labeled, containing the date and time of application and the name of the person who is authorized to use it, and the reason for the tag-out or lockout (repair or maintenance, for example). It should be clear that these devices are not able to be removed until a full LOTO procedure is followed.
